Rate limit the public API
Add rate limiting to the public API: 60 requests a minute per key. Clients should know when to retry.
What changed- The limiter keeps one bucket per key, in memory.
- The middleware runs after auth, so it limits keys, not addresses.
- All 42 tests pass, five of them new, one for the burst Codex found.
